Call, Duty Hours, Supervision and the 2011 ACGME Standards

The ACGME approved new duty hour guidelines on September 28, 2010.  The final standards went into effect on July 1, 2011.  Please note that we were already compliant with these rules. On weekdays, 2 residents are in house from 5PM to midnight, one at the St. Luke's site and one at the Roosevelt site. From midnight to 8AM, there is one resident in house from midnight to 8AM at the St. Luke's site (also covering the Roosevelt site by PACS).
 
On weekends and holidays, 2 residents are in house from 8AM to midnight, one at the St. Luke's site and one at the Rossevelt site. From midnight to 8AM, there is one resident in house from midnight to 8AM at the St. Luke's site (also covering the Roosevelt site by PACS).

Residents are supervised by 2 in house faculty (one at each site) on weekdays until 11PM. There is one in house faculty from 8AM to 8PM on weekends and holidays. (Residents and faculty can interpret and review cases from either site using our PACS system.) A faculty member works off-site from midnight to 8AM from Sun through Thursday and is available for consultation. At other times, faculty are on call.

First year (R1) residents take the following call:

R1 residents rotate through the vascular interventional service and take VIR call. R1 residents also take call during the evening and weekends in preparation for more independent call in the R2 -R4 years. 

R2 -R4 residents take the following call:

The Roosevelt evening shift is 5 to 6 days, with a total work week of 46 to 54 hours. (Sometimes, residents will take the 6th day, Friday as a separate shift.) Residents take 9-10 weeks of this call during their entire residency.

The St. Luke's night float rotation is 5 to 6 days, with a work week of 60 to 71 hours. (Sometimes, residents will take the 6th day, Friday as a separate shift.)  Residents take 9-10 weeks of this call during their entire residency.

These schedules are within the maximum 80 hour work week mandated by the ACGME and New York State. They are also compliant with the ACGME 6 evening in a row limit.
